 If someone wanted to Shoot or Fight a target that was grappling with a third party, what would be the effects and/or penalties be?

I would, as a consequence of a miss, let the Fighter/Shooter hit the grappler, damage him with weapon damage. But I wouldn't penalize the skill roll itself.
