Is there a mechanic we missed?

My group was playing out a "Danger Room" slugfest with the Quickstart characters and Monster Girl managed to kill Invincible with an improbably good roll.
She scored six 6's, his block scored none. She poured all her stunts into bonus damages, so Invincible took a total of 20 points after accounting for his armor of 4.
Invincible's 13 health dropped to zero with 7 left over.
The Critical Hit die also came up a 6. Invincible's "Lucky Break" Talent, let him re-roll it, but he scored 5, for a total of 12: Head Crushed.
Everyone at the table was a bit dumbfounded and convinced we must have done something wrong, but we can't figure out what. Is there some kind of mechanic that can prevent someone from dying or somesuch?
Please note that this isn't a complaint, we just want to make sure we didn't screw something up by not reading the rules closely enough.

You can't pack more than one extra success into the same stunt against the same target. See page 84, under "Slugfest Stunts":
Choose one stunt from the list below for each extra rolled. Each stunt may only be chosen once for each target.
So, in the case of your game, Monster Girl's player could only use one of those five extra successes for Extra Damage; the other four would have to be distribued among other stunts. This means she would have done 4 points of damage after Invincible's armor was factored in.

Note that there's a Deadly Hit stunt that forces a roll on the Critical Injury table, but it requires the power or weapon to have the "sharp" modifier, which Monster Girl doesn't have.
