If your Fighting or Agility is at 12 would you still get the copy bonus of 3+ from Duplication for a total of 15 dice for attacking?

Same for Agility 12 and Evasive. Would you get 14 dice to dodge or would you cap at 12?

There is no hard cap for the maximum number of dice you can roll. In your examples, you would indeed roll 15 and 14 dice respectively!
