Does Gear take damage from failed dice (i.e. rolled a one) always, or only on a push roll (like the fails on Base Dice)?

All failed skill roll will have a consequence. It is up to you a a GM to decide what that is. You can as a consequence let gear take damage, but besides that, gear only takes damage if your roll Explo on the gear dice during a pushed skill roll.
