Basic combat questions:

If during a turn I use all my actions (2) to attack an orc, but he survivies. When is the orc turn to attack, can I defend myself? i.e. dodge or parry?

You must have an action available in order to dodge or parry. So if you have used both your actions, then no, you can't dodge or parry (unless you use a free dodge or parry from a talent).

Further, unless you have a talent that allows you to attack with your fast action, you can't attack with 'both actions' as they are different kinds of actions with different kinds of costs. Fast actions cannot be used for attack barring a talent.

Actually, you could consider a SHOVE or DISARM action an attack, in which case, you could do one of these actions and an ordinary attack, for a total of two. I believe that's what the OP mentioned.

Well there’s no attack option as such. It’s slash/stab/punch(kick/bite).

So it’s not that you can only “attack once” it’s that the above are slow actions (usually) so you’re limited in how many you can do by slow/fast economy. A “grapple attack” is a fast action so you can do two of those normally.
