Also, don't forget that the rest of the party has agency as well. They can do something to turn the tide, and perhaps distract or taunt the enemy, or even grapple with or disarm them, thus buying time for the broken pc to crawl away.

In what way does that negate the fact that it's a feels bad mechanic? What you are suggesting specifically strips agency out of the players hand. They have to sit back and watch to see if they die or not. Anything that takes agency out of the players hand to a point that it results in death is bad....

The idea that the player looses a fight to a point where they are broken, spend experience on talents to help them survive, and then have that Exp come to nothing because the DM decided mid fight to execute the player anyway, at the behest of a dice rol...

That is not generally something you, the gm, should be doing though. The players cannot fail forward and the story has nowhere to go if npcs are just executing them mid fight.
Simple. A d6 is rolled, and if a 1 is the result...

With this, at least the sense of danger is always felt.

Nope, that's what quickdraw is for =)

Although, generaly if they just want to take a couple of steps I'll let them do that as a free action.
Speaking of Quickdraw, would you allow a player to draw and parry on an enemy's turn?

Just remembered that an NPC could also perform a coup de grace. Lucky can't stop that.

By extension, this would presume that monsters and unintelligent beings can choose to end the PC's life on their turn, Lucky or not.
